Athletic Training

On behalf of Jackson College and Henry Ford Allegiance Sport Medicine, I would like to welcome you to Jackson College Athletics. Sports Medicine coverage here at Jackson College is contracted through Allegiance Sport Medicine and consists of a team physician and a full-time Certified Athletic Trainer. Our focus is to address the sports medicine needs of each student-athlete through prevention, evaluation, management, rehabilitation, and referral of athletic injuries and/or illnesses. Our primary concern is to maintain the health and safety of every student-athlete while participating in Jackson College athletics.

The following documents are Essential in our efforts to provide optimal healthcare for Jackson College student-athletes.

  1. Insurance Information Form - Please fill out form completely and attach a copy of your insurance card(s) (front & back). Please be aware that Jackson College requires each student-athlete to have Primary Health Insurance. Student-athletes that do not return Insurance Information Form or provide proof of existing insurance coverage will not be able to participate in any practices or games until they do so.
  2. Athletic Pre-Participation Physical Examination Form - Please answer ALL medical history questions honestly and accurately. Please be certain to provide signature(s) on bottom of physical form. This form can be completed with your primary care physician or at the Jackson College Health Clinic.
  3. Medical Consent to Treat and Release of Information Form - Please read form carefully, then print, sign and date form.
  4. Sickle Cell Trait Form
  5. Athletic Health and Wellness History Form

Please ensure that all forms are fully completed and legible. It is imperative that all information is accurate and that all appropriate signatures are included. Please bring all completed forms with you to your designated physical date listed above.

Secondary insurance policy through First Agency

The first policy has a $0 deductible and with a $15,000 medical maximum. This coverage has a 52 week benefit period. All services must be completed within the 52 weeks of the injury to be considered for coverage. The second policy has a $15,000 deductible and with a $10,000 medical maximum. This coverage has a 104 week benefit period. Coverage will not get into the second policy until the $15,000 deductible is satisfied. As an example, if you had an injury and only $10,000 in bills were incurred in the first 52 weeks that is all the first policy will consider.  If more services for the same injury result in bills, these services would not be considered until the $15,000 deductible is satisfied. Once this deductible is satisfied, then any bills beyond that would be covered.

Student-Athlete Drug Testing Information

The Jackson College Department of Intercollegiate Athletics sees participation in intercollegiate athletics to be a privilege and is committed to providing student-athletes with the opportunity to participate in intercollegiate athletics in a safe environment. The Athletic Department also believes the abuse of legal drugs or the use of illegal drugs can adversely affect the mental and physical well-being of student-athletes. In an effort to help increase the physical and academic performance of student-athletes and protect the health of student-athletes and integrity of its athletic programs the following policy will be followed by the Jackson College athletic department. 

The purpose of the Athletic Department drug testing policy is to educate student-athletes regarding the health and safety hazards inherent with the abuse and misuse of drugs.  In addition, deter students from the potential abuse, misuse or experimentation with drugs and other physically or psychologically harmful substances and identify student-athletes in need of early intervention for possible drug use or addiction. The Athletic Department want to promote fairness and integrity of intercollegiate athletics at Jackson College and provide student-athletes with a reason to “say no” to drug use through education.
